Dietary confirmation

You can request a dietary confirmation for the tax authorities if you follow and follow a diet prescribed by a general practitioner, doctor-specialist or dietitian for allergic food hypersensitivity or celiac disease. A dietary confirmation is also possible with a diet at underweight or lactose intolerance and as of 2016 also with the FODMAP diet.

A dietary statement is charged 15 minutes of consultation time. I can only issue a dietary confirmation if you are in treatment with me.
